Competitors who wish to register by post will need to submit a range of application forms depending on their status as a Competitor, Non-Playing Coach, Technical Support Personnel or Accompanying Person. You can use this page to download the forms you will need.

Competitors submitting their applications by post will need to submit the following forms:
- Sport Entry Form (download form)
- Release of Liability and Acknowledgement of Rules and Regulations (download form)
- Team Registration Form/Roster (to be submitted by Team Captains only) (download form)
- Personnel Information Form (download form)
- Competitors who wish to enter boxing must provide a Medical Release signed by a licensed physician. See (General Rules) for more details

Non-Playing Coaches submitting their applications by post will need to submit the following forms:

Games Transport Passes, Dinners & Ceremonies
Accompanying Persons, Non-Playing Coaches, Technical Support Officers and Competitors who wish to book Games Transport Passes, tickets for the Gala or Welcome Dinner and tickets for the Opening/Closing Ceremonies must complete a Personal Information Form.
